Leadership Camp Registration

I, Parent Parent, warrant and represent that I am the parent or lawfully appointed guardian of Participant First Name Participant Last Name (the "Participant").

I hereby give permission for the Participant to attend and participate in the Archdiocesan Leadership Camp (the “Camp”) to be held at Glen Harbour Kids Camp between August 10-16, 2025.

In consideration of the Participant being permitted to attend and participate in the Camp, and for other good and valuable consideration, the receipt and sufficiency of which is acknowledged, for the Participant and myself, I hereby release and discharge the Archiepiscopal Corporation of Regina, and all of their respective directors, officers, employees, religious, and volunteers, of and from any and all liability, whether in negligence, pursuant to statute or otherwise for any loss, damage, expense or injury, Including death, that the Participant may suffer as a result, directly or indirectly, in whole or in part, of attending or participating in the Camp.

I warrant and represent that I am not relying upon any oral or written representations or statements made by or behalf of the Archiepiscopal Corporation of Regina with respect to the safety of the Camp.

I voluntarily agree to accept transportation for my child to and from the Camp on a licensed charter bus.

Photos/videos will be taken during this event. I understand that these photos/videos are the property of the photographer/vidoegrapher, and he/she may edit, alter, copy, exhibit, publish or distribute this photo/video for purposes of publicizing the Archdiocese of Regina or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of these images. The Archdiocese of Regina has consent to use my likeness in any and all of its promotions or publications, including website and social media, without payment or any other consideration. The Archdiocese of Regina will not trade, share or sell to any outside party and content will remain the property of the photographer/videographer. I hereby certify that I am the person or parent/guardian of the person named above and do hereby give my consent without reservation to the above.

This release and covenant not to sue shall be effective and binding on my personal representatives, heirs and assigns, and those of the Participant.
